About the job
Provides remote, patient-centered nurse care management by guiding adult members through complex medical and behavioral health needs, care plans, treatment, and benefits.
What you'll do
- Deliver care management by telephone or video, including longitudinal follow-up and support during acute episodes.
- Create care plans with members and the care team, and coordinate with local providers and community resources.
- Help members navigate conditions, treatment pathways, benefits, and the healthcare system.
