About the job
Provides telephonic and video care management for adult members, coordinating clinical care, benefits, providers, and resources through complex and acute health situations.
What you'll do
- Create care plans with members and the clinical team, and support members in achieving care goals.
- Help members navigate medical conditions, treatment pathways, benefits, and healthcare services; coordinate with local providers.
- Provide longitudinal follow-up and case management during acute episodes, including hospitalizations and rehabilitation stays.
