About the job
Provides virtual, longitudinal nursing care management for newborn and pediatric members, coordinating clinical care, benefits, providers, and resources to support health outcomes.
What you'll do
- Deliver coordinated care by telephone or video and create care plans with members and the multidisciplinary team.
- Guide members through medical conditions, treatment, benefits, and healthcare navigation; coordinate with local providers.
- Provide follow-up and case management during acute episodes, including hospitalizations and rehabilitation stays.
