About the job
Provides remote, telephone- and video-based care management for adults with complex medical and behavioral health needs, coordinating care plans, providers, benefits, and resources.
What you'll do
- Deliver coordinated virtual care management and create care plans with members and the care team.
- Help members navigate conditions, treatment, benefits, and healthcare services; coordinate with local providers.
- Provide longitudinal follow-up and case management during acute episodes, including hospitalizations and rehabilitation.
