About the job
Provides telephonic and video oncology care management, coordinating clinical and social resources and guiding members through complex care and treatment journeys.
What you'll do
- Deliver coordinated virtual care management and develop care plans with members and the care team.
- Help members navigate medical conditions, treatment pathways, benefits, and the healthcare system; coordinate with local providers.
- Provide longitudinal follow-up and case management during acute episodes, including hospitalizations and rehabilitation stays.
