Provides individualized, patient- and family-centered nursing care, education, and care coordination for patients at Jefferson Health’s cancer center.
What you'll do
- Develop and deliver individualized plans of care, patient education, and discharge planning in partnership with patients and their designated care partners.
- Coordinate with interprofessional teams, follow nursing protocols, and use evidence-based practice and information technology to support safe care.
- Participate in quality and safety improvement, committees, and staff precepting.
