Leads inpatient care management operations across assigned acute care sites, improving care coordination, discharge planning, patient transitions, and departmental performance.

- Oversees departmental operations, staffing, budgets, quality initiatives, and care management performance across multiple facilities.
- Directs care coordination, discharge planning, utilization management, and transitions of care in collaboration with clinical and community partners.
- Uses data and process improvement initiatives to strengthen quality, compliance, efficiency, and patient outcomes.
