Leads and supports Community Health Workers while connecting clients with health care and community resources to improve health outcomes and address social needs.
What you'll do
- Train, mentor, and coach CHWs in resource navigation, healthy behaviors, and navigating health care systems.
- Develop training and workflow improvements with managers and directors, and provide oversight and support at community events.
- Manage a modified client caseload, document services, and serve as an expert on Health Related Social Needs assessments and EHR workflows.
