About the job
Provides nursing home visits and support to pregnant women and their children up to age 2 in the Nurse Family Partnership program.
What you'll do
- Provide comprehensive nursing services and follow the nursing process and NFP home-visitation model.
- Manage a caseload of up to 28 clients; provide education, support, and referrals to help families reach their goals.
- Complete all required NFP training.

- Current Pennsylvania RN license in good standing.
- Valid Pennsylvania driver’s license, current automobile insurance, and reliable vehicle for use on duty.
- Excellent written and verbal communication skills and basic computer skills.
