Coordinates individualized cancer care, guiding patients and families through diagnosis, treatment decisions, referrals, symptom management, and transitions across the care continuum.
What you'll do
- Coordinate diagnostic work-up, results communication, referrals, follow-up, and care plans with patients and multidisciplinary teams.
- Educate patients and caregivers about cancer, treatment expectations, disease processes, and potential side effects; support treatment decision-making.
- Support Patient Navigators and multidisciplinary colleagues, attend Tumor Boards, and help manage symptoms and transitions to reduce emergency visits and readmissions.
