About the job
Provides longitudinal, comprehensive primary care to adults through telehealth, coordinating with interdisciplinary teams and managing preventive, acute, and chronic care needs.
What you'll do
- Conduct telehealth visits, assess patients, order and interpret diagnostics, formulate care plans, and prescribe appropriate interventions.
- Document and bill care in Epic, review results and patient messages, and coordinate referrals, follow-up, and cross-coverage.
- Collaborate with interdisciplinary teams to address care gaps, quality goals, patient access, and outcomes.
