Provides individualized, patient- and family-centered nursing care, including assessment, care planning, education, treatment coordination, and quality and safety improvement.
What you'll do
- Develop and coordinate individualized plans of care with patients and designated care partners, respecting their needs and cultural beliefs.
- Assess patients, administer care and education, and support discharge planning and follow-up.
