About the job
Provides individualized telemetry nursing care, education, and care coordination for patients in partnership with families and the interprofessional team.
What you'll do
- Assess patients, develop and implement individualized nursing care plans, administer medications, and plan for discharge.
- Educate patients and care partners, communicate and coordinate with care teams, and use information technology and outcome data to support safe, high-quality care.
- Participate in quality and safety improvement, committees, professional development, and precepting appropriate staff.
