Provides nursing assessment, interventions, education, triage, and care coordination for patients in an ambulatory clinic, collaborating with providers and support staff.
What you'll do
- Deliver evidence-based nursing care during clinic visits, telephone encounters, and electronic communications; educate patients on preventive care, chronic disease, medications, and common illnesses.
- Coordinate care, including triage, abnormal results, vaccination and injection verification, and inbox management; document care in the electronic medical record.
- Collaborate with the clinical team and participate in care pathways, quality improvement, and safe, patient-centered care.
