Provides ambulatory nursing care, patient education, triage, care coordination, and clinical support for a primary care team.
What you'll do
- Provides evidence-based nursing interventions and education for preventive care, chronic conditions, medication management, and common illnesses.
- Coordinates patient care, including triage, abnormal results, vaccinations, referrals, prior authorizations, and forms.
- Documents care in the electronic medical record and supports quality improvement and safe, coordinated team-based care.
