Coordinates inpatient care and discharge transitions for patients, developing individualized plans and working with patients, families, and multidisciplinary teams to support safe, timely, cost-effective care.
What you'll do
- Assess patient needs and develop care and transition plans with the multidisciplinary team.
- Coordinate discharge arrangements, referrals, authorizations, transfers, and resolution of barriers to care progression.
