What you'll do
- Support local Clinically Integrated Network participation, partnerships, referral relationships, and site readiness.
- Help recruit and select the inaugural care team and provide clinical leadership and mentorship to APCs.
- Provide in-person and virtual adult primary care, with a focus on obesity and metabolic health, and coordinate with the care team.
