What you'll do
- Coordinate care across inpatient, outpatient, emergency, and home-care settings; develop personalized care plans with members and providers.
- Assess social needs, connect members with community resources, and help resolve barriers to care.
- Educate and advocate for members, including providing crisis intervention and guidance on health conditions, discharges, procedures, and prescriptions.
