About the job
Provides remote, patient-centered obstetric and maternal-child nursing care, including assessment, care planning, triage, and follow-up.
What you'll do
- Assess patients, create individualized care plans, set wellness goals, and address barriers to care.
- Monitor diagnostic results, triage urgent concerns, educate patients on remote monitoring equipment, and reduce care gaps through follow-up.
- Document care, escalate concerns to multidisciplinary teams, and provide feedback on programs and products.
