Provides remote, patient-centered obstetric and maternal-child nursing care, including assessment, care planning, triage, and follow-up.
What you'll do
- Assess patients, develop individualized care plans, and set wellness goals based on evidence-based standards.
- Monitor diagnostic results, address urgent concerns, and triage patients to appropriate services.
- Document care, reduce care gaps through patient engagement, and share feedback to support program and product development.
