About the job
Makes outbound and inbound calls to educate patients about Pomelo Care and enroll them in its services, while supporting follow-up and care coordination.
What you'll do
- Make high-volume outbound calls, explain available clinical and social resources, and meet enrollment goals.
- Answer patient questions, document outreach, and help schedule appointments and follow-up care.
- Coordinate with healthcare providers, community organizations, and other stakeholders to connect patients with appropriate services.

Pay
