About the job
Provides occupational therapy for PACE participants in San Gabriel Valley, assessing functional abilities and developing interventions to support independence, safety, and well-being.
What you'll do
- Assess participants and create individualized treatment plans in collaboration with the Interdisciplinary Team.
- Provide therapy interventions, equipment recommendations, home safety assessments, and caregiver education; monitor progress and adjust plans.
- Document care and coordinate with participants, families, clinicians, and external providers.

- Graduate of an accredited occupational therapy program; Master's degree in Occupational Therapy required.
- Valid and active California Occupational Therapy license.
- Proficiency in English and Mandarin and/or Cantonese.
- Ability to commute to Alhambra.
