About the job
Provides personal care and daily living support to PACE participants at the adult day center or in home care, while helping maintain their safety, comfort, and well-being.
What you'll do
- Assist participants with ADLs and IADLs, including bathing, dressing, feeding, transfers, and ambulation.
- Maintain a clean and safe center or home, support food service and activities, and escort participants as needed.
- Document services, communicate changes or complaints, and follow participant rights, confidentiality, and safety procedures.
