About the job
Provides holistic primary and preventive care to older adults in Seen Health’s PACE program, coordinating with interdisciplinary teams, families, and technology and operations staff.
What you'll do
- Deliver culturally sensitive primary, preventive, acute, and chronic care, including home visits as needed.
- Conduct health assessments and develop individualized care plans with participants, families, and the interdisciplinary team.
- Coordinate specialty and discharge care, participate in team meetings, and help improve care workflows and quality.
