Supports a caseload of older adult participants by providing psychosocial support, coordinating care and community services, and assisting Social Workers and the interdisciplinary team with documentation and administration.
What you'll do
- Provide participant and family support, deliver and document care-plan interventions, and report changes in participants’ conditions or psychosocial status.
- Coordinate community resources, care transitions, discharge planning, visits, and end-of-life support.
- Assist Social Workers and the interdisciplinary team with care reviews, meetings, reports, and required documentation.
