About the job
Plans and delivers psychosocial care, counseling, case management, and care coordination for PACE participants and their caregivers as part of an interdisciplinary team.
What you'll do
- Assess participants’ psychosocial needs and develop, implement, and update care plans.
- Provide counseling, case management, referrals, care transitions, and coordination with families, providers, and community agencies.
- Document services, support behavioral management and discharge planning, and respond to elder abuse reports.
