Provides after-hours clinical support and care navigation, including crisis assessment, safety planning, referrals, and coordination of mental health services.
What you'll do
- Support members on a clinical phone queue using clinical engagement approaches, including active listening and motivational interviewing.
- Assess risk, provide crisis support and safety planning, and follow up with high-risk members.
- Recommend and coordinate appropriate care, document interactions, and collaborate with providers and internal teams.
