Provides overnight clinical support and care navigation to members seeking mental health, substance use, or relationship support, including crisis assessment and referrals.
What you'll do
- Provide clinical phone support, psychoeducation, crisis assessment, safety planning, and follow-up with high-risk members.
- Recommend care plans and coordinate referrals to Spring Health providers or external resources.
- Document interactions and collaborate with providers and multidisciplinary teams to support members.
