Support members through mental health care coordination, clinical assessment, crisis response, and connection to appropriate services in English and Spanish.
What you'll do
- Manage care coordination, including platform support, intake, appointments, and explaining available services.
- Assess clinical needs, refer members to appropriate care, and provide crisis triage, safety planning, and follow-up.
- Document member interactions in English and coordinate with internal teams to resolve barriers to care.
